ABSTRACT
What follows is a copy of the Group/Life History Questionnaire history form that I have all prospective group members complete and bring with them to our initial screening session. This form is designed as a template. It is not intended to fit every group leader’s specific practice. Although the level of detail invited in this form is not appropriate for many of the groups discussed in this book, the following section is included to give readers an idea of the kinds of questions discussed during screening and preparation sessions for traditional psychotherapy groups. As you will see, it is not appropriate for work-based or non-support focused groups. Feel free to skip ahead. Or view it as a smorgasbord with items that might fit when screening for your group.
