ABSTRACT
An educational intake form sample document is included in this guide. This form was created in PDF and can be done in print or digital format. This intake form is utilized by the education staff (ES) when a youth file is created, and a youth is on an ES caseload for the purposes of educational planning or monitoring. If the notification came from child welfare, utilize the child welfare educational planning procedures and adapt to fit the county specific needs. If referral/notification came from juvenile justice, utilize educational planning procedures specific to juvenile justice youth. When records are obtained and interviews conducted, the ES decides the level of services necessary to ensure educational success. The ES should not be put in a punitive position so as to recommend consequences for the youth for failure to participate in educational services.
