ABSTRACT

A readiness assessment utilizes quantitative and qualitative data to provide both the breadth and depth of the required understanding to design and implement a Connected Communities of Care (CCC). The Clinical Advisory Group members will consider health and social data based on prevalence rates of chronic diseases in the health system and community at large as well as data from surveys and interviews with clinical and Community-Based Organization leaders. As potential users of a CCC platform, clinical and community stakeholders play a significant role in guiding its design. The readiness assessment interviews and focus groups should foster a rich exchange of information on potential use cases to be addressed through the CCC model as well as barriers and enablers to using the technology. For these use cases, the Partners will discuss examples of how the CCC may play a role in coordinating transitions of care, reducing redundancies and inefficiencies, and improving clinical and social outcomes.
