ABSTRACT

Connected Communities of Care (CCC) is designed to engage healthcare providers and Community-Based Social Service Organizations aka Community-Based Organizations in a combined approach to deliver holistic care that addresses an individual’s clinical and social needs. The requirements to establish a model for sustainability include the following: seek initial funding, secure ongoing funding to ensure financial sustainability, consider operational sustainability in the initial CCC design and implementation and regularly re-examine and refine the elements of the CCC, including relationships with Partners, to ensure operational sustainability. The Sponsor of a successful CCC will need to research the local, regional, and possibly national philanthropic organizations in order to potentially tap into their particular interests, which may generate CCC funding. In addition to a focus on financial sustainability, CCC Sponsors and the governance entity should emphasize the operational sustainability of a CCC, meaning ensuring the CCC can continue to operate in an effective and efficient manner despite unforeseen challenges.
