ABSTRACT

Humans, individually and collectively, tend to be reactive and respond to problems where and when they arise. The main issues that are recognised challenges today – among them productivity, quality, safety, reliability – did not occur at the same time but appeared successively over many decades. Each issue was addressed when it arose and independently of the others, following the divide and rule principle that can be found everywhere in politics, warfare, and business, resulting in a fragmented view and a fragmented approach to change management. This chapter describes how the concerns for productivity, quality, safety, and reliability came about and how each has left a legacy for how we manage organisations today.
