ABSTRACT

Formal Safety Assessment (FSA) is a designation used in a number of different contexts and industries (e.g., the nuclear industry) to describe a rational and systematic risk-based approach for safety assessment. In the maritime world the expression Formal Safety Assessment (FSA) is being used by International Maritime Organization (IMO) to describe an important part of the rule-making process for international shipping. The FSA approach is described in a series of steps, covering preparations, hazard identification, risk assessment, identification of safety measures, cost-benefit assessment, and finally recommendations for decision-making. The chapter describes these steps in detail, providing examples at various steps. Toward the end, a comprehensive example is included.
