ABSTRACT

An important part of safety management is to be able to learn from accidents, failures, errors, and mistakes. This chapter briefly describes relevant regulations and moves on to describe some of the most common accident theories that have been proposed and that form the basis for understanding accidents. Several accident investigation methods are described, including STEP, MTO method, and M-SCAT. Further, the practical performance of an accident investigation is described in six steps. The content of an accident report is also described, providing guidance on how to structure the report and the content of each section. At the end, a list of sources of accident investigation reports is included.
