ABSTRACT

Whilst PBS plans are now incorporated into our routine work, not all of the information obtained from our shared assessment and formulation process lend themselves to PBS plans. Rather, a range of care planning processes and templates is needed to fully support the range of care plans that can result from a shared assessment and formulation process. We highlight the areas of assessment and formulation we identify for potential care planning purposes and describe in detail a number of methods for care planning through the use of case studies.

All of our care plans have the shared goal of reducing barriers and enabling recovery and are based on a shared understanding of the service user’s unique inner world. This ensures the clear identification of treatment targets based on their life and recovery goals in an effort to increase social participation. Regular reviews of the impact of care plans through the routine use of our formulation templates and other tools can enhance the planning of care by assessing the effectiveness of the interventions. This grounds the care provided in a psychologically informed and systematically evaluated, service user-focussed way.
