ABSTRACT

This chapter focuses on supporting people in making plans for and initiating changes in activity and participation patterns. There are several types of changes that can be planned – adding new activities, building on previous activities, constructing opportunities that are aligned with health and well-being benefits, and opportunities that facilitate inclusion and citizenship. The chapter describes challenges that can be anticipated in implementing planned changes. Attention is drawn to the assumptions and biases held by service providers and systems that can impact supported change. The chapter is organized into the following sections: types of changes to activity and participation patterns; identifying and prioritizing plans for change; anticipating challenges and implementing supported change; building on the momentum of change; attending to assumptions, judgements, and biases about activities and participation; and key competencies.
