ABSTRACT

The potential benefits of using the AC-W change model on its own or in tandem with other models are shown through the entire change process. The authors consulted with colleagues in these case studies to assess their emotional and cognitive readiness for change to aid the implementation of these service changes. Case study 1 combines the use of the AC-W model and Lewin’s three-stage Change Management Model. It illustrates a prospective change, the implementation of an App for the self-management of type 2 diabetes in primary care and community, involving three GP practices and three medical wards in the local acute NHS hospital Trust. Case study 2 demonstrates the use of the AC-W change model in a retrospective change, the redesign of an acute medical unit into a medical division. Here we show the extent of the preparedness of people and resources for that change, its implementation and ongoing review. Case study 3 uses both the AC-W model and Kotter’s eight-step change model in a real time change to implement an innovative scheme to attract and retain staff in mental health, both within the acute mental health Trust and primary care community mental health services.
