The Insurance Superbill Must Have Your Name as the Provider

A recent newspaper article reported the story of a social worker who spent the night in jail. She didn’t pull off a bank job nor did she hold up an armored car. Instead she put somebody else’s name as the provider on the insurance bill she gave to the client because the insurance carrier in her state would not reimburse a social worker’s services.
