ABSTRACT

Let history and the past be a teacher to organization personnel for improving organizations' responses during emergencies and outages. Organizations' current practices and adherence to the minimum code of operation should be able to give their management and personnel confidence in their ability to deal with any emergency or crisis. Any incident or event should be analyzed to know "what went well and what did not?" It is the "what did not go well" that should be discussed further to understand areas for improvement. These would include missing capabilities, what should be changed in the current practice, etc.
