It is essential when teaching these exercises that they are used as a starting point with expressions such as “the first rung on the rehabilitation ladder”. Abdominal hollowing was traditionally said to be a key action for core stability training. The kneeling core training position can be progressed using limb movement. Limb movements of the leg or arm are used in exercise types such as Pilates. In addition, the crook lying position can progress to trunk curl actions or V sit exercises. As we move from the reactive phase to the recovery phase our aim is to enable the person to return to their chosen activities pain free, confident, and without limitation. The roll down is a useful exercise to introduce flexion in a controlled manner from a standing position and is often practised as a Pilates movement. The Jefferson curl is a weighted roll down which works both the spinal musculature and overloads the hamstrings in the final position.
