ABSTRACT
Traditional HCO care models are presented in brick-and-mortar settings and home healthcare, whereas digital health models break down location and distance barriers and offer several modalities for the provision and receiving of care – telehealth, home healthcare, at-home intelligent monitors, and brick-and-mortar settings are used to develop hybrid workflows across multi-disciplinary teams.
