ABSTRACT

In this chapter, the authors focus on how to prevent problems, but remember that many of the activities are shared with problem solving. Making changes to processes, systems, and even strategies is very common in every organization, but thinking about the future isn’t so commonplace. In this chapter, the authors explore basic ways to prevent problems, rather than trying to solve them after they appear. Like problem-solving, there are a series of steps, or a roadmap, that can be followed to successfully analyze and hopefully prevent future potential problems. A causal chain analysis for the bolt head thickness being too high consists of two main paths, each of which results in identifying the corresponding root causes and corrective actions. Being observant, alert, and watchful during and after the implementation will help ensure the plan's success.
