ABSTRACT

It is acknowledged that healthy lifestyles are as relevant for individuals diagnosed with chronic conditions as they are for individuals with no diagnosed condition. Recommending and supporting lifestyle change for individuals with a chronic condition requires health practitioners to consider any particular risks and associated amendments to standard advice that different conditions indicate. For some conditions there are specialist lifestyle support courses available. This chapter considers four different chronic conditions: Type 2 diabetes; coronary heart disease; mental health and chronic obstructive pulmonary disease.
