It is helpful to give the questionnaire a heading, if only for one's own convenience, for example in the follow-up. Do not repeat explanations given in the covering letter, except for those regarding confidentiality or anonymity. However, if there is no covering letter then briefly explain the reason for the survey. All the instructions needed to complete and return the questionnaire should be on the questionnaire itself rather than in the covering letter. Instructions for completing the first item should be given. When the method of responding changes, give new instructions. When an item may not be applicable to the respondent, allow for this possibility in one's instructions, asking the respondent to skip any items which do not apply. If you are inviting respondents to give their names for possible followup interviews or clarification of doubts, do this at the end. Finally, thank them for their help and explain how they should return the completed questionnaire.
