ABSTRACT

International Civil Aviation Organization's Dan Maurino has attacked the culture of traditional accident investigators who, with few exceptions, have not considered it a part of their task to investigate the human factors aspect of their investigations. Many thoughtful critics maintain that traditional accident analysis in the US, and in many other parts of the world, seems more interested in finding fault or blame than in preventing accidents. A theory of air transport accident/incident causation that is widely supported by many experts is the concept of the 'error chain'. 'The Significant Accident Causes and the 'Flight Crew Causes and Their Percentage of Presence in 93 Major Accidents' are given in Appendix M. Professor Reason has pioneered a new organizational framework that involves a total system approach to accident investigation. Some have argued that management is at least partially implicated every time air carrier personnel are identified as the 'primary cause' of an air transport accident.
