ABSTRACT

There are numerous considerations that must be evaluated before an investigation can formally be started. Every organization will have their reporting relationships established for the investigator to include authority, updates, written reporting and distribution and financial approvals if needed.

In simplistic cases such as an immediate loss or incident there may not be many considerations and the field investigation will quickly evolve into a formal investigation based on the circumstances and obvious evidence presented initially. The following are all considerations that should be evaluated, if applicable, before starting a major and sometimes a minor investigation that may impact operations.
