ABSTRACT
The “just culture” model is a powerful effort to limit use of the no-blame model. It allows limited inquiry into systemic problems, but blocks the deeper inquiries that reveal basic systemic flaws. The advantages of pursuing a no-blame system model with no limits, and no impediments to systemic inquiries, are clear; and the resistance to such unlimited inquiries is one of the major challenges to implementing the no-blame system.
