ABSTRACT
Questions in this section relate to the client’s medical history and current health. The aim of the questions is to collate information on health issues that may affect or compromise voice production, such as respiratory disorders, ear, nose and throat (ENT) disorders, previous surgery, disorders affecting posture or movement, allergies or medication.
