ABSTRACT
Safety management currently faces a number of issues across industry, including plateauing performance, poor perception and repetitive failures particularly apparent through reoccurrence of major accidents. Businesses are doomed to continue repeating the same failure unless they can approach safety management in a way that results in a different outcome and breaks the cycle of repetition.
