ABSTRACT

Effective roadmaps actually serve as beacons or markers that help innovators navigate their way without being distracted and thrown off course. Once the digital innovation roadmap had been established, the big challenge was how to execute such a large-scale digital transformation program with agility and speed. This chapter describes the transformative innovation deployed within the University of Kentucky Healthcare system that created a dynamic financial catalyst, which expanded patient access and improved patient outcomes. This innovation was built upon the framework of ambulatory pharmacy services and grew to represent 70% of Income from Operations for the health system while reducing readmissions by 40% and expanding access for insured and uninsured patients. The chapter explores how patient access was expanded, how the fiscal impact was measured, the barriers circumvented and lessons learned were applied to other departments. A growing chasm between inpatient and outpatient pharmacy operations is being bridged with transitions of care models.
