ABSTRACT
For many years, and for many people in health care today, long-term care has meant nursing horne care. That definition is no longer valid. Although we may not yet be able to boast of a coordinated system of chronic care in most communities, we have advanced considerably since 1965, when the Older Americans Act, Medicare and Medicaid laws were passed, providing the stimulus for development of a coordinated system of chronic care.
