ABSTRACT
Effective reporting and monitoring of incidents are identified by Poyner and Warne1 as the single most important factor in attempting to make workplaces and staff safer.
The under-reporting of violent incidents is a significant problem in monitoring such attacks and developing responses, and its importance cannot be over-emphasized. Unless there are accurate records, the case for additional resources for developing a safety strategy is difficult to make.
