Chronic disease

In primary care, the majority of consultations are for chronic disease. The Quality

Outcomes Framework (QOF), introduced in 2003 in the new General Medical

Services (GMS) contract for primary care in the UK, is a significant advance. This

ensures that regular surveillance and review is undertaken to monitor and treat a

number of defined common chronic illnesses and disease.
