ABSTRACT

Tools You Can Use contains a variety of forms that can be customized to meet a school or school system’s needs. In Part II, eight sets of roles and responsibilities were presented. The associated forms appear in this section. In some cases the same form works regardless of the professional assignment. In other cases, two forms are presented, one for classroom teachers and the other for resource/specialty area teachers. The table below shows the various forms provided within this section as well as those that have been modified for classroom teacher-5 domains, classroom teacher-4 domains, and resource/specialty area teacher. In some cases multiple versions of the form have been created for professionals to select the format they prefer or the applicable target audience. A few forms work regardless of the teacher’s position.
