The Chinese Communist Party (CCP) under the leadership of Mao Zedong undertook the immediate task to rebuild the nation after the end of Second Sino-Japanese War (1937-45) and civil war (1947-9). It made health a priority and regarded the health care system as a major pillar of state socialism (Gu 2001: 197). This chapter provides important historical context necessary for understanding health insurance reform in China by examining the development and characteristics of public health system, health care delivery and financing systems in urban and rural areas during Mao’s era (1949-76). It also examines how the centrally planned health care system was shaped by the complex interplay of forces, namely institutions, historical legacy, interests and ideologies. It shows that the development of health care carried political, ideological and diplomatic significance in the pre-reform era.
