ABSTRACT

Every seven seconds someone, somewhere around the world is involved in an accident that will change their life. Accidents occur as a result of weaknesses in the control systems in place within an organization. These controls include procedures, physical measures and employee behaviour. Corrective actions are the silver linings waiting to be uncovered by the process of accident investigation. Carefully identifying actions that will effectively reduce the risk of similar events recurring and implementing them appropriately improve the reliability of one's control systems for the future and move forward on his journey to zero accidents. Two common ones utilized around the globe, the first from the United States of America: An accident is an undesired event that results in personal injury or property damage. And now from the workplace safety regulator in the United Kingdom: An accident is a separate, identifiable, unintended incident, which causes physical injury.
