ABSTRACT
As this work draws to a close, I know we are all interested in finding out who the culprits were, although it should be somewhat evident at this stage; how they did what they did and got away with it, at least up to a certain point; and what motivated their actions in the first place. Also, where should final accountability lie, in the circumstances?
