ABSTRACT

This chapter draws together the main findings of the evaluation of the Lewisham Case Management Scheme (LCMS) and considers some of the implications for the development of care management for older people with dementia and their carers in the context of policy initiatives to promote integrated health and social care provision and the personalisation of care. The Production of Welfare (POW) approach provides the conceptual framework for the research methodology for all the studies. The Lewisham study offers important insights regarding three current concerns about the development of services and the implementation of policy. These relate to the sustainability of innovatory service models, personalisation and the deployment of personal budgets, and the roll out of the National Dementia Strategy. A striking feature of the Old Age Mental Health Services in Lewisham is the coherence and logic of the organisational model within which the service operates and the staff undertakes their roles.
