ABSTRACT

This chapter focuses supporting and encouraging the client to implement and maintain change and support self-management strategies. It also discusses the ongoing relationship where people might continue to review, monitor and evaluate the client's progress with them. There are a number of transition models, and people may want to explore more of these in their further studies, but chapter explore a couple of the transition models that are helpful to understand the change process more fully. Hopson and Adams seven-stage model process is not linear, and clients move backwards and forwards between the various stages of transition. Cycle of change model has stages relating to the experience of change, as in Hopson and Adams, but the model sees the change process as cyclical rather than linear. ABCDE model is useful in challenging those beliefs, testing the accuracy of the statements and replacing them with more preferential statements.
