ABSTRACT

The Early Years Foundation Stage (EYFS) profile, completed for all children in the final term before they turn five, will provide vital information on those who have received Special Educational Needs (SEN) support in the Early Years setting. Education, Health and Care (EHC) and SEN Support Reviews should include planning and preparation for transition to another setting or school. The special educational needs and disability (SEND) Code of Practice explains that Local Authorities (LA) must ensure that the EHC Plan Review at Year 9, and every review thereafter, includes a focus on preparing for adulthood. The challenge of the transition plan lies in the development of continuity of assessment, review and programme planning from school through further educational, vocational and personal preparation for a valued and productive adult life. Transition is part of growing up for all children and young people with SEND and will require planning and support.
