ABSTRACT

Neither the ICD-10 nor the DSM-IV provide guidance for treatment or prognosis in first-episode psychosis. I have found it practical to consider that one is faced with one of three possible disease courses. Roughly speaking, they each encompass a third of those with a first-episode psychosis. I shall briefly describe the clinical pictures. Naturally the future course is never possible to predict with certainty. This three-type grouping is not intended to be a new diagnostic tool but an aid in the clinical discussions about treatment and prognosis.
