ABSTRACT

This book is about what happened and what is going on, rather than about detailed theoretical argument. The main reason for this is that such finer details are only of real interest to a minority - those who have had psychological training of one kind or another. My other books were mainly written for them, and contain quite a lot of scholarly material which can be chewed over in the usual academic way. But in reading over this book, I have become aware that there are certain theoretical points which perhaps need to be clarified, because they are referred to every now and again without any real explanation being given, and this may be frustrating to the reader. So I'd like to try to mop up at least a few of these points here.
