ABSTRACT

Having completed the PPS, the practitioner is confronted with issues of documentation. Behavioral health practitioners are quite well versed in writing evaluation reports, so that PPS documentation may seem to be a simple matter, at first glance. However, documenting the evaluation of a spine surgery candidate carries unique challenges, as well as responsibilities. The practitioner who attends to these documentation issues will produce reports that are, at once comprehensive, succinct, scientifically based and respectful of the patient's need for privacy.
