ABSTRACT

A sample set of ‘customer satisfaction’ survey forms is included in the appendix. The forms provide a compromise between the highly structured models already widely used throughout education and the unstructured model, described in the text, which leaves the choice of issue to the customer. The model described here provides some structure, which was appreciated by students who found the write-in sections rather onerous, combined with the option to customize. Most of the forms contain the following:
