ABSTRACT
Documentation is vital in order to promote a high level of patient care. The Nursing and Midwifery Council’s (2009) Record Keeping: Guidance for Nurses and Midwives states that good record keeping is an integral part of nursing and midwifery practice and is essential to the provision of safe and effective care. These principles must be followed in order to ensure patient notes are maintained to a good standard.
