When a patient presents with a pressure ulcer at Grades 2, 3 and 4, it is a reportable incident and it is essential that an incident form is completed. This may be used in a court of law so it is important to ensure the facts are correct. Each trust will have its own policy on incident reporting, so make sure you are aware.
