ABSTRACT

All healthcare professionals whether in the community setting, residential homes or hospital must work together in ensuring pressure ulcers are managed effectively. Unfortunately, due to referral issues and a lack of multidisciplinary communication during patient admittance and discharge, patients at risk of pressure ulcer development or those patients with a current pressure ulcer sometimes fail to receive the urgent treatment they require. This can then result in prevention being delayed and skin breakdown and may also result in a patient with a pressure ulcer missing treatment. Sometimes a lack of training and knowledge can also be a contributory factor. This needs to be addressed as a matter of urgency by the managers and all staff must attend tissue viability modules to gain the knowledge and competence they require.
