ABSTRACT

Welfare services in the United Kingdom provide a rich but often incoherent and incomplete range of support for people and their families. There is a bewildering array of sectors and services that may be relevant which could be provided by public, voluntary and/or private organisations. The main aim of integrated care is to ensure that people and their families experience coherent support that is focused on their needs and aspirations. Coordination is an important element of quality care and support being provided within an organisation. The world health organization describes four domains of continuity: interpersonal, longitudinal, management and informational. Care coordination seeks to address the common lack of communication and organisation between agencies and professionals. People with severe and enduring mental health problems may have periods in their lives in which they are in receipt of support from multiple professionals and services.
