The title should indicate very briefly the overall topic of the questionnaire. If you will be surveying individuals who are interested in the topic, the title may grab their attention and increase your response rate. Example 7 .1.1 is the title for a survey on the health of new mothers. It is poor because it is not sufficiently descriptive-in fact, the key term in it, CAREDATA, is jargon whose precise meaning is known only to those working in the organization conducting the survey.
