ABSTRACT

The technology infrastructure for an information exchange platform, such as the Connected Communities of Care (CCC), is a critical component enabling the stakeholders to achieve clinical and social goals to fulfill the CCC requirements. The CCC should facilitate technology support for the Participants and users of the CCC technology platform. For CCC site users and their environments, there are associated factors that can impact adoption and widespread use of the technology platform. The Technology Platform Track lead will reconcile the technology requirements with existing and short-term planned features of the CCC technology platform. The technology platform should provide closed-loop referrals, meaning that the referring entity receives information on the outcome of the referral from the entity that received the referral. The technology platform should ensure that secure communication about a person can be shared between the referral source and the Community-Based Organization as part of the referral process and feedback loop.
